Sports Physical: Sports physical are typically valid for three years. Fine Arts do not require a sports physical. (i.e. Speech, Jazz Band, Drama, etc.) Your student’s sports physical on record can be verified on Parent Portal (see Student’s Name, Health Tab).
Updated Sports Physicals can be submitted to the ZHS Activities Office or emailed to jillayne.reinking@isd728.org. You cannot practice without a current sports physical on file. MSHSL Sports Physical Form
Co-Curricular Fee Payment Options: Pay with Registration or Pay at a Later Date. If you select to “Pay at a Later Date” the Fee will be available for payment on the Parent Portal “Fee Tab” the following business day. If you will meet the family max ($775.00 ~ please note this does not include Middle School or Youth Sports) please choose Pay at a Later Date and then contact Jill Reinking.
If you qualify for financial assistance you must complete the form each year and submit a copy of the letter to the Activities Department to have your fee waived or reduced. Educational Benefit Link
Insurance: ISD 728 does not provide any type of health or accident insurance for injuries incurred by your student at school. Please see link for options for supplement health insurance.
Practice/Tryouts: All teams start date is Monday, August 12, 2024. Time TBD by each team so please check social media or contact the coach.
Please contact the Head Coach or Advisor for sport specific information. i.e. Practice/Try-outs schedules, equipment needed etc.
