Online Registration
Please register early and allow up to 72 hours for the Activities Office to process your registration. Last minute registrations may not be able to be processed in time before practices start. Until registration is complete, there is no participation.
CLICK HERE TO BEGIN YOUR REGISTRATION!
- Login to Parent Portal (parent/guardian, not student)
- Click on School Store
- Choose the school your student attends
- Click on Athletics or Activity
- Select the Sport/Activity you are registering for
- Select Recipient and Activity Option (pay now or pay later)
- Click on Required Forms (26-27 Athletic Registration Packet) Complete & Sign
- Submit Forms
- Add to Cart
- Select cart and submit for payment (even if you are selecting pay at a later date) Your registration WILL NOT be received by the Activities Office if the steps bolded in red are skipped.
EDUCATIONAL BENEFITS APPLICATION
Co-Curricular Fee Payment Options: Pay with Registration or "Pay at a Later Date". If you select to “Pay at a Later Date” the Fee will be available for payment on the Parent Portal “Fee Tab” the following business day.
Please note that the ISD 728 Family Maximum per school year is $1,000 (MS sports excluded from this calculation). Contact the Activities Office if you have reached or are approaching that amount. Please note this does not include Middle School or Youth Sports. Please choose "Pay at a Later Date" and then contact Jill Reinking.
Once your cart has been successfully submitted, you will receive an email stating, "Thank you for registering for an ISD728 Activity”. If you do not receive a confirmation email, your registration has not been submitted successfully. Please try again.

HOMESCHOOL Students
Click HERE to download the Activity/Athletics Registration Packet. After this is completed, Save as a PDF. Email the packet to (jillayne.reinking@isd728.org).
Required - Sports Physical
Athletes participating in MSHSL sanctioned sports (grades 7-12) must have a current sports qualifying physical exam on file prior to the first day of practice. Sport physicals are good for three years from date of your last physical, and must remain current throughout the entire sports season. Drama, Jazz Band, Knowledge Bowl, Speech and Trap DO NOT require a physical.
Once your physical form has been completed by your doctor, email a copy to jillayne.reinking@isd728.org or scan and submit the completed physical forms online through your family account.
Physical Dates on file can be verified on Parent Portal: (Students Name, Health Tab). Without a valid physical, students can NOT practice.

Please have your physician complete, sign, and date the form. Then submit to ZHS Activities Office.
If a student gets injured, a medical note will be required to return to participation in sports.
Insurance: ISD 728 does not provide any type of health or accident insurance for injuries incurred by your student at school. Please see the link for options for supplement health insurance:
